Build me a form builder and response collector I host myself.

STACK
- Astro with a Node adapter, SQLite via better-sqlite3, server-rendered HTML
- Local disk or S3-compatible storage for uploads

FORM DEFINITION
- A form is a JSON file: fields with type, label, required, help text and validation
- Types: text, long text, email, number, date, select, multi-select, file, rating
- Conditional visibility as a rule referencing an earlier field's value
- Changing a form never rewrites past responses: store the form version with each submission

THE PUBLIC FORM
- One page, server-rendered, fully working with JavaScript disabled
- Client-side validation as an enhancement only; the server validates again and is the authority
- Spam defence without a captcha: a honeypot field, a minimum time-to-submit, and a per-IP hourly limit

FILE UPLOADS
- Size and MIME allow-list enforced server-side by reading the magic bytes, not the extension
- Randomised storage names, never the uploaded filename, and no directory listing

RESPONSES
- Append-only table; an edit is a new row referencing the previous one
- CSV export that quotes correctly and a JSON export that keeps types
- A webhook per form, retried with backoff and a visible dead-letter list

NOTIFICATIONS
- Email to the owner on submit, and an optional receipt to the respondent

Include a README covering the upload directory permissions, the backup of both database and files, and how to add a field type.
